How to register

The Practice's area stretches from post code districts Liverpool 3 to Liverpool 15 and we welcome new patients.

Step 1: Check you are within the practice catchment area. Click on the following link and enter your postode to see if you are eligable to register with us.

registerpatientsStep 2: Download the following documents from the bottom of the page or Visit the practice to get the following: Registration Form (GMS1), New Patient Questionnaire and an Alcohol Consuption Questionnaire. 

Please note: You will need to bring you Passport or other Photographic ID and proof of address to register with the practice.

Step 3: Complete Registration Form. The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Primary Care Trust to quickly trace your medical records and send them to your new surgery. The New patient questionnaire will ask you for some basic information about lifestyle and ethnicity, so we can provide the best service possible from the start.

Step 4: Attend new patient check.

It is important that you attend your appointment with the nurse for a new patient check. She will take a medical history from and enter any significant information such as allergies and medications onto your computer record.

Step 5: Placed on the Practice list. Once you have completed and signed the form and attended your new patient check, you will be registered with the practice. Shortly after this COM will send you a new medical card with your doctor's name on for you to keep.
